BOAT REGISTRATION APPLICATION (Form 02-841) is required for an original boat registration transaction or when there is a change in ownership. DMV uses both a single-part application form and a two-part application form. The single-part application form is used in offices where the Certificate of Number is issued across the counter. The two-part application may be used by dealers who are not on-line. These offices do not assign boat numbers or issue Certificates of Number. The second copy of the application may be used as a temporary registration for up to 60 days allowing use of the boat until DMV issues the Certificate of Number.
Businesses that have fleets of boats have raised concerns about the difficulty in accomplishing the initial registration. This applies mainly to the need to fill out a separate application for each boat when the owner name, address, and much of the boat information is the same. To accommodate these concerns DMV offices may accept a single signed application with the generic information such as name and address and then an attached sheet with a list of the boats, the HIN (if any) and the boat description (length, color, etc).
The data entry screens in ALVIN closely follow the sequence on the application. The letter codes on the various multiple choice data fields on the application are the same letter code that is entered into ALVIN for that particular field. (Example: “B. Cabin Cruiser” is checked for the “Boat Type” on the application. Enter “B” in the “Boat Type” field in ALVIN.)
Following is an explanation for every section of the application:
- PRESENT AK BOAT NUMBER - The Boat Number for a boat previously registered in Alaska. The number may have been assigned by the Coast Guard or by DMV. The number always begins with “AK” which is preprinted on the application. Boats that have not been registered in Alaska will not have an assigned number so this field will be left blank. When a new boat number is assigned, enter this information and circle it to show it was entered by a DMV employee or representative.
- REASON FOR APPLICATION – Check the appropriate box for the transaction. “NEW” means a new registration in Alaska. This may be a new boat, a boat transferring from another state, or a boat that has been in the state but is being registered for the first time.
- BOAT NAME (IF APPLICABLE) – This is an optional field. The boat name is an aid mainly for search and rescue operations.
- CLASS OF BOAT – This field will determine the class code, depending on if a powered boat or non-powered (optional, except Sport Fishing Guides) boat is being registered. B1 = powered and B2 = non-powered.
- HULL IDENTIFICATION NUMBER (HIN) – Manufacturers of recreational boats have been required to issue a 12 digit letter/number combination HIN since 1972. If the boat does not have a HIN, ALVIN will assign a HIN. (B-110)
- ADF&G # (IF ANY) – A permit number issued only to commercial fishing boats by the Alaska Department of Fish & Game.
- LENGTH (FT) – The overall length of the boat rounded up or down to the nearest foot. A boat that is 25.5 feet would be rounded up to 26 feet. A boat that is 26.49 feet would be rounded down to 26 feet. DO NOT USE METERS. To convert meters to feet, multiply the number of meters by 3.281. _______ (meters) x 3.281 = _____ (feet)
- YEAR - The year the boat was built. The last two digits of the HIN are usually the year the boat was built, but may be the year the boat was first registered in Alaska.
- MAKE/BUILDER OF BOAT – The boat manufacturer. DMV uses codes provided by the National Crime Information Center (NCIC). See Appendix B. If the manufacturer is unknown but the boat does have a HIN, the make may be determined by matching the appropriate letters in the HIN to the Manufacturers Identification Code (MIC). The MIC is a unique code assigned to each manufacturer by the Coast Guard.
- BOAT TYPE – The type of boat. If none of the choices match the boat type, use “OTHER” and write in a description. For example: Hovercraft
- PROPULSION – Identifies the type of propulsion. If the propulsion is an auxiliary means of propulsion it should be identified. “D. SAIL ONLY” and “E. OARS/PADDLES” should only be used when there is no other means of propulsion and no intention of putting other means of propulsion on the boat. These two categories are considered non-powered boats that have a lower registration fee.
- USE – This identifies the use of the boat. If a boat is used for both commercial and recreational use, commercial should be entered, regardless of the percentage of commercial use.
- FUEL TYPE – The type of fuel used, if applicable.
- HULL MATERIAL – The primary material used in the hull.
- HULL COLOR - The primary hull color.
- TRIM COLOR - The primary trim color, if applicable.
- CABIN COLOR - The primary cabin color if different than the hull color.
- CONTACT TELEPHONE NUMBER – Daytime phone number of the primary owner.
- MAILING ADDRESS – The primary mailing address for the owner(s). This is where any correspondence, such as renewal notices, will be sent.
- RESIDENCE ADDRESS - The primary residence address of the owner(s) of the boat.
- OWNER – The names of up to four individuals, businesses, or other entities may be listed. Use an “OR” conjunction on all transactions, as boats are not titled in Alaska and any owner may change the record.
- SOCIAL SECURITY NUMBER (SSN) – The SSN of each owner. The SSN will be used to link the names in the driver and vehicle data bases. The SSN will not be released as a part of the public boat record. This field is optional.
- DOB – The date of birth for each owner. Not applicable to boats registered in the name of a business.
- DRIVER LICENSE # - The driver license number of each owner. Not applicable to boats registered in the name of a business.
- PRINTED NAME AND SIGNATURE OF APPLICANT/DATE – One of the owners must sign and date the application. DMV will accept an application in which the dealer has signed for the owner for phone and mail sales. The application should be signed "owner's name by salesperson's name, company".
- DMV USE ONLY – This section is to be completed by the DMV employee/agent processing the transaction as follows:
- PREV. STATE – The last state the boat was registered in.
- STICKER NUMBER – The control number on the Validation Sticker. If no sticker is issued, leave blank.
- BATCH – The batch number the transaction is processed in.
- AMVC – The three-digit logon ID of the person that processed the transaction.
- DOCS. ACCEPTED: - Circle the name for the document(s) being accepted.

- CARP. CERT. – Carpenter’s Certificate
- OWNER AFF. – Affidavit of Boat Ownership (Form 848)
- MCO – Manufactures Certificate of Origin. Make a copy. Give the original to the owner.
- BOS – Bill of Sale
- TITLE – Title. Make a copy. Give the original to the owner.
- FEES – Circle the amount of the fee collected.
- PAYMENT TYPE: - Circle the code for the payment type. CA = Cash; CC = Credit Card; CK = Check
- DATE – The date the transaction is processed.
