Alaska law states that owners of motor vehicles shall apply for title and registration. The "Application for Title and Registration" (Form 812 or 812A) is the document the Division provides to a person who is applying for a title and registration for a vehicle or trailer. This form enables the Division to collect all the information that is needed to create or make changes to a vehicle record. Other documents are required in addition to the application; however, the application allows us to look at most of the required information on one piece of paper.

This Standard Operating Procedure (SOP) provides step-by-step instructions for completing the application.

There are two vehicle manuals to assist you when processing requests for a title (SOP T) and/or registration (SOP R). As a quick reference, an example of form 812 is included as a link, showing where to easily find specific information needed to complete each section of the application.

The application is a legal document and becomes a permanent part of the motor vehicle history file. Some important things to remember when accepting an application:

  • Every application must be signed in black or blue ink by the applicant(s). (i.e.: pencil is unacceptable)
    See T-150 for Signature Requirements
    Exception: For applications for snow vehicle registration (NTI), a fax is acceptable.
  • Any statement in the "affidavit" section must be in ink.
  • All Division entries must be made on the right hand side of the application in the "For Division Use Only" section. A DMV representative may circle or highlight information on the application but cannot alter, change, or add information.

The application may be a one-part form (Form 812) or a three-part form (Form 812A). Form 812A is used for requests from dealerships and lending institutions and is distributed as follows after the transaction has been completed:

  • Original - retained by the Division and scanned as part of the record
  • Yellow - applicant's copy*
  • Pink - held in the processing office or returned to Dealer/Fleet, if applicable

*The yellow copy of the application can be validated by a DMV representative and is the receipt for fees, temporary registration and proof of application for the vehicle owner. This procedure may apply during computer down time. (A-210)

INSTRUCTIONS AND INFORMATION FOR COMPLETION OF APPLICATION

SECTION 1 – VEHICLE

SERIAL NUMBER / VEHICLE IDENTIFICATION NUMBER (VIN)

The correct and complete VIN is required. The VIN must be obtained from either a prior title or a Manufacturer’s Certificate of Origin (MCO) when a title is being issued. For registration transactions, the VIN may be obtained from a copy of a prior Alaska title or registration, a copy of a foreign certificate of title, a non-negotiable title, or a foreign registration. Spaces, signs and symbols (/, #, --, &, etc.) may not be used in the serial number. Unless a character can be clearly recognized as the letter "O" or "I" use the number zero (0) or one (1).

Beginning in 1981, all manufactures have assigned a 17 digit VIN to new vehicles. An application showing a VIN of less than 17 digits for a 1981 or newer model year vehicle requires a VIN inspection prior to issuance of a title.

To correct a VIN, the following is required:

  • The complete and correct VIN must be entered in Section 1.
  • The incorrect VIN is entered in Section 6.
  • Refer to T-100, "Title Corrections, Additions, and Deletions", for additional requirements.

YEAR

The model year of the vehicle is entered in this section.

Example: A vehicle manufactured in 1981 is entered as "1981".

When more than one year is shown on a foreign title, enter the vehicle MODEL year. When the year of a vehicle is unknown or cannot be determined from supporting documents, check the NICB (National Insurance Crime Bureau) or NATB (National Automotive Theft Bureau) manual to determine the model year. A VIN is required to research Division records. If the model year is not available, use the following guidelines:

  • RECONSTRUCTED VEHICLE (T-270) - Use the year the vehicle most visually resembles.

    Example: A vehicle constructed from a 1953 frame and a 1976 body will be assigned the model year of 1976. Use the VIN from the 1953 frame.

  • HOMEBUILT TRAILER (T-260) - Use the year construction was completed as the model year.
  • OTHER TRAILERS - An inspection may be required. If the model year is not identified, have the applicant estimate the model year in the affidavit portion of the application, or use the year it was first registered.
  • ALL OTHER VEHICLES - The vehicle must be inspected. If the year is not identified, estimate the year based on body style, model, and year first registered. If you have access to an NATB book you may refer to it for help in identifying vehicle year model.

MAKE - The name of the manufacturer in entered in this space. In ALVIN, enter the NCIC code for the manufacturer's identification, such as Ford (FORD), Chevrolet (CHEV), or Subaru (SUBA). The "make" must be the same as shown on the Manufacturer's Certificate of Origin (MCO). If you do not know the code, check the table in ALVIN. To do this, go to the General Menu and select function 10 (Display Specific Table Value). Use “100” for the Table Number or “PR-VEHMK” for the Table Name. Enter the manufacturer’s name in the Table Value field. If there is not a code listed for the manufacturer, contact Headquarters (Appendix A) for assistance. (See instructions on the Employee site for sending an email request for a make code.)


MODEL - This is a name given to different styles of a particular make. The customer would enter the model name; however, you would enter the NCIC code in ALVIN. To find the NCIC code for the model, go to the General Menu and select function 10 (Display Specific Table Value). Use “101” for the Table Number or “PR-MKMO” for the Table Name. Enter the NCIC make code in the Table Value field. Select the model code from the list that is displayed. You may need to press enter a few times to get to the model code you are looking for. Do not reject the application if the vehicle model is not available. When the model appears on supporting documents, it must appear on the application.

Example: Enter FORD and look for “Explorer”. “XPL” is the model code.

BODY STYLE - The vehicle body style is that which most accurately resembles the physical appearance of the vehicle.

  Example: 2 Door (2D) 4 Door (4D) Hatchback (HB) Motorcycle (MC)

Acceptable body styles are located in Appendix C.

NOTE: "Chassis Cab" is generally not an acceptable body style. This type of vehicle is sold by the manufacturer as complete without the rear component. Such vehicles are used to add camping units, etc. The body style is that which is visually identifiable upon completion. If the chassis cab has not been modified, the body style "Chassis Cab" (CB) may be used.

Make every effort to obtain the color of the vehicle. For vehicles with more than one color, use the dominant color.

IS VEHICLE USED COMMERCIALLY?

When an applicant selects “No”, use a non-commercial class code. Exception: Vehicle’s titled in a business name and trucks/vans over 10,000 pounds unladen weight.

“Yes” is used for vehicles titled in a business name, buses, leased vehicles, and passenger vehicles that are used commercially. Trucks/vans over 10,000 pounds unladen weight are automatically classed commercially regardless of how the customer completes the application.

See R-105 for further clarification on commercial use.

UNLADEN WEIGHT (POUNDS)

The unladen (empty or shipping) weight of a vehicle is required for trucks (including pickups and vans), buses, and all vehicles used commercially. The registration fees for these vehicles are based on empty weight. The application is rejected without the weight for these vehicles. The vehicle weight for all other vehicles is entered when available. The applicant may estimate the weight for non-commercial vehicles. It is the customer’s responsibility to provide the vehicle weight. If the customer estimates the weight and they are incorrect, they are not eligible for a refund. They can correct the weight and it would be correct for the next renewal.

ODOMETER (MILES) (T-25)

Federal regulations require a vehicle buyer to acknowledge the vehicle odometer reading. The odometer reading must be in miles. Record only full miles. Example: 35,905.6 would be entered as 35,905.

AXLES, USDOT, TAX ID NUMBER (TIN) (R-108)

This information is required for all commercial use vehicles that require a USDOT number.


SECONDARY SERIAL NUMBER - The complete, correct motorcycle engine (motor) number is entered for motorcycles and motor scooters, when available. This section is also used to record corrections to a motorcycle engine number. If making a correction enter the correct engine number. A statement must be made in the affidavit section defining the reason the engine number is changed and identify the previous or incorrect number.

The unit number of a motor home or a second serial number on a reconstructed vehicle may be entered as a secondary serial number.

UNIT NUMBER - This information is entered as a courtesy to our fleet customers and does not affect anything else in the record. Maximum number of digits is seven (7).

SECTION 2 – OWNERSHIP

The full names (first, middle and last) of all owners of the vehicle must be entered on the application in order to appear on the title [AS 28.10.041(d)]. If the person does not have a middle name or has an initial but not a middle name, have the customer make this statement in the affidavit section. When there are more than two owners, the applicant will need to list any additional names in the affidavit section. Up to four names can be on a title; however, the registration will show only the first two names. See SPECIFIC REQUIREMENTS on the following pages to determine what is acceptable to use in the Name/Company Name fields.

The applicant would place an "X" in the box marked "Owner" and “Co-owner”, if applicable. For leased vehicles, the applicant would mark both the “Lessor” and the “Lessee” boxes.

OWNER NAME (INDIVIDUAL) - The applicant must enter their full first name, middle name, and last name. If the customer enters NMN (no middle name), do NOT enter this in ALVIN. A statement is not required if the customer enters NMN, as this is self-explanatory.


OWNER INFORMATION (USED FOR INDIVIDUALS) - The applicant should circle YES or NO for the Organ Donor question. This does not appear on Form 812A, as the applicant must be in front of a DMV representative in order for us to enter this information. See D-060 for information on how to enter organ donor information when the applicant is not applying for a driver license. The applicant’s driver license number, date of birth, social security number, and sex is entered on the second line. This information is OPTIONAL. NOTE: When entering this information in ALVIN, only an Alaska driver license number would be entered as ALVIN can link the rest. If an applicant would like to be able to update the address on the vehicle record using our online change of address system, the driver license MUST be recorded in the vehicle record in order to be eligible for this service.

COMPANY NAME (BUSINESS, LESSOR, TRUSTS) - When the owner is a business, leasing company or trust (T-390), the applicant uses this field to enter the name.

CONJUNCTION - When there is more than one owner, the applicant must select a conjunction by placing an "X" in one of the boxes. Selection of “AND” requires the signature of ALL owners in order to sell/transfer the vehicle.

Selection of “OR” permits just one of the vehicle owners to sell or transfer the vehicle. Only one conjunction can be used when there are more than two owners. (i.e., cannot use: Linda Jones OR Mary Olson AND Joanne Wilson. In this case, the conjunction would be AND between all owners.)

SPECIFIC REQUIREMENTS:

NEW VEHICLE - The owner may select whether the individual’s name(s) or a business name will appear on the title. However, the applicant's signature (T-150) must be identical in content with how the name(s) appear on the MCO and other supporting documents succeeding the MCO. In the event the signature is different, an affidavit must be provided stating why the applicant's signature is other than what appears on the MCO. This statement may only be made by the person(s) identified on the MCO.

Example: The MCO shows Dianne Denver. The application shows "Designs by Dianne". A statement must be made by Dianne Denver that she wishes to title and register the vehicle in her business name. She must sign the application using the company name and signature line.

USED VEHICLE:
  • Title Issued - The owner’s name or names on the title/registration must be the same name(s) as appears on the assignment of the prior Alaska or foreign title. Additional names may be added by the vehicle owner/applicant, provided the application is signed by a person shown as the buyer on the previous ownership document.
  • No Title Issued (NTI) - “Registration Only” - The owner name(s) on the Application for Title and Registration (812) must be the full name(s) of the vehicle owner, regardless of the name on the out-of-state registration.

ACCEPTABLE TO USE FOR OWNERSHIP:

  • Lessor (LSR) and Lessee (LSE) - Leased Vehicles. (T-280) Enter the actual name of the leasing company or individual. Enter “R” for the Lessor and “S” for Lessee for the ownership in ALVIN and do not use a conjunction.
  • Name Titles (Jr., Sr., I, II, III, IV, V, etc.)
  • Professional business name (PC after name)
    Example: Tammy Thompson MD PC (Commercial use)
  • Trusts (T-390) - The actual name of the trust is acceptable. Example: Jones Family Trust or John Jones or Leslie Murray


UNACCEPTABLE TO USE FOR OWNERSHIP:

  • DBA (Doing Business As) - Enter just the actual name of the business.
  • JROS (Joint Rights of Survivorship) and WROS (With Rights of Survivorship)
  • Personal Titles (Mr., Mrs., Ms.)
  • Professional titles (Dr. PhD, DDS, etc.)
  • Trustee - The name of an individual with “trustee” after it is not acceptable. Example: John Jones, trustee or Leslie Murray, trustee

SECTION 3 -- MAILING ADDRESS OF OWNER OR LESSOR

The complete mailing address of the vehicle owner(s) is entered in this space. The address of either owner may be used when the vehicle is jointly owned. This will normally be an address in Alaska, but could also be an out of state address.

SECTION 4 -- PHYSICAL ADDRESS OF OWNER OR LESSEE

In order to title and/or register a vehicle in Alaska, either the vehicle must be physically present in the state or the vehicle must be owned by a resident of Alaska. The residence (physical) address is required by 02 AAC 92.210, AS 28.10.081, AS 28.10.231 and AS 28.10.431(e) and is used to determine applicability for various programs such as the Inspection and Maintenance (I/M or emissions program) (R-430) and Motor Vehicle Registration Tax (MVRT) (R-190). Normally an Alaska address will be given; however, there are circumstances that warrant using an address outside of Alaska. (See the additional information below for dual registration, active duty military and college students.) If a street address or legal description is not available, the best location identifier (i.e. milepost, campsite, lot and block number, etc.) is acceptable. In the case of a transient or a person who lives in a motor home, the residence location on the day of application may be used.

Post Office Box Numbers and SR Box Numbers are not acceptable. Mail Receiving Stations are not acceptable for the residence address.

Dual Registration (R-345) – Commercial vehicles may be issued a registration only (NTI) for a vehicle that is currently registered in another state or jurisdiction. These will have the address of the other state or jurisdiction.

Military - Alaska residents must provide a copy of a current (within 90 days) Leave and Earning Statement (LES) showing Alaska as their home of record in order to use an out-of-state address on an original application for title and registration.

Students - Alaska residents must provide a letter from the school registrar's office verifying that they are full-time students in order to use an out-of-state address.

SECTION 5 -- LIENHOLDER AND MAILING ADDRESS

The name and mailing address of the primary lienholder is entered in section 5. The word "none" must be entered if there is not a lien on the vehicle. (N/A is not acceptable in this section.) In cases where there is more than one lien on a vehicle, we only record the primary lien.

The owner/lessor and the lienholder cannot be the same individual or business. In this case, the name/business is recorded as the owner or lessor and the lien is recorded as “None”.

SECTION 6 -- TO CORRECT A SERIAL NUMBER (VIN)

When a title has been issued with an incorrect VIN and you want to make a correction, the applicant must write the correct number in the vehicle information area under “Serial Number (VIN)” and enter the incorrect VIN in section 6. It's a good idea to circle or highlight this section once it has been completed so that the correction is not overlooked during a title transfer.

SECTION 7 -- IF PURCHASED FROM DEALER

An application received from a Dealer may or may not have this information included. If this portion is left blank, do not reject the application.

SECTION 8 – DONATION

If the applicant has selected "Yes" to a monetary donation and has given a donation amount, enter the information using the guidelines in D-60.


SECTION 9 – AFFIDAVIT

This section is used when there is a transaction in which the data on the application is not self-explanatory or if additional information is required to complete a transaction. All entries in section 9 must be in ink or typewritten. All requests for duplicate registration, replacement plates and/or tabs must be supported with an explanation in this affidavit section. The explanation should indicate the reason for the request for the duplicates. When a commercial vehicle subject to PRISM requirements is not required to have a USDOT number, an affidavit is required.  Example: "DOT number not required".  A Vehicle Transaction Application (Form 821) may be used for transactions when a title is not being issued.

SIGNATURE OF OWNER (T-150)

Any transaction involving an Alaska title requires the signature of the vehicle owner(s) on the application. For leased vehicles, the signature may be from the lessor or the person leasing the vehicle. A registration renewal may be requested by a person other than the owner. In this case, the person must first provide us with the personal information contained on the record, due to the privacy act.

  • Title Transaction (Including NTI) - Request that the applicant review all the information on the application before signing. The signature on the application affirms that the information is true and correct. The applicant must sign in ink as the name appears in Section 2. A signature can be signed using a power of attorney (T-130).
  • Registration Transactions (Excluding NTI) - If the person applying for registration renewal is not the registered owner the applicant would sign as shown in the following example.

Example: If Charles Carlson were applying for a registration renewal for Susan Dey’s car he would sign the application: Charles Carlson for Susan Dey

FOR DIVISION USE ONLY:

ACTION

The boxes provided in this section identify what type of transaction the applicant is requesting. An "X" must be entered in only one box.

  • Title and Registration - Transactions that will result in a title and registration being issued, including temporary permits (T-90) for vehicles not previously titled/registered in Alaska.
  • Title Only (T-80) - When issuing a title only to vehicles not currently registered in Alaska and not currently being registered or renewed.
  • Original N.T.I. (registration only) (T-50) - We do not title Snow vehicles, ATV's (R-135) or boats (B SOP). In addition, we issue a registration only (NTI) for vehicles that have never been titled or registered in Alaska when the original title is being held by a lienholder outside of Alaska. Also see Dual registration (R-345).

For the following transactions, a Vehicle Transaction Application (Form 821) can be used, provided the vehicle owner has an Alaska title and/or registration in their name. Write in the type of transaction if the customer has already completed form 812.

  • Change of registration month
  • Duplicate Registration
  • Duplicate Title (Form 821 is not needed unless there is a change of address or lienholder.)
  • Plate transfer (Permitted for the following: Alaska Commemorative Veteran, Amateur Radio/Call letter, Charitable or Non-Profit Exempt, Children’s Trust, Disabled Veteran, Government Exempt, Disability Plates, Personalized, Prisoner of War, Purple Heart, Support our Troops, Gold Star Family, University and Veteran plates)
  • Registration correction (color, weight, class, plate number, month/year tabs)
  • Registration Renewal
  • Remove lienholder
  • Special plate request

LICENSE / DECAL / PERMIT NUMBER

Enter only the Alaska license plate number, snow vehicle/ATV decal number, or temporary permit number presently assigned to the vehicle. Leave this space blank if the vehicle is being titled or registered in Alaska for the first time.

If the last issued Alaska registration has been lost or not available, the license plate number must be verified through ALVIN , microfiche or the research retrieval system.

IF REQUESTING PLATE TRANSFER, ETC. - When a plate transfer is requested, enter the VIN for the previous vehicle. A completed Certificate of Eligibility and copy of FCC license must accompany transfer application for Amateur Radio Plates. (R-300) The owner name must match on both the current and previous records. If the previous vehicle was sold, the ownership information can be found in the title history.

OLD EXPIRATION DATE - Enter the month and year the last ALASKA registration expires/expired.

NEW EXPIRATION DATE - Enter the month and year the vehicle registration or temporary permit will expire. Snow vehicle and ATV registrations all expire September 30th. Refer to R-175, Determining registration month.

NEW LICENSE/DECAL/PERMIT - Enter the following in this box, when applicable:

  • Lost License Plate: Enter the new license plate number that was issued. Note: Be sure to issue new month and year tabs as shown on current records.
  • Original Registration: Enter the new license plate number.
  • Snow Vehicle/ATV Decal: Enter the new decal number.
  • Temporary Permit: Enter the temporary permit number.
  • License Plate Transfer: Enter the license plate number being transferred.
  • Title Only to Title & Registration: Enter the new license plate number, if applicable.

TAB NUMBER ISSUED - Enter the year and the full number, including the "alpha" character, of the tab being issued for these transactions:

  • Original registration
  • Renew registration
  • Replace plates and/or tabs

CODES

  • Class: Enter the two digit class code determined from the appropriate R SOP. For vehicles previously registered in ALASKA, check to assure that the usage has not changed. (from commercial to non-commercial or visa versa; personalized or special issue plates , etc.)
  • Tax Location: (R-190) Enter the tax location code, if applicable. Be sure to remove “S” (Senior), “M” (out of state military resident), "D" (disability) or “A” (outside of taxable area) designators from ALVIN when transferring ownership.
  • Inspection: (R-430) Enter “N” if the customer resides outside an I/M area. If the customer lives in an I/M area, enter the correct I/M code. (Y, E1, E2, E3, E4, E5)
  • Previous State: Enter the two letter abbreviation for the state or foreign jurisdiction where the vehicle was previously titled or registered. If the vehicle is new enter NEW or double asterisks (**). Enter “AK” if an Alaska title is surrendered. Acceptable codes can be found in Appendix A, or display (Function 12) Table Number “249” or Table Name “AD-STNAM” from the General Menu.

FEES - Note: ALL fees are entered in whole dollar amounts. ($68.00 is entered as 68)

  • Registration: Enter the amount of the vehicle registration fee as determined from the appropriate R SOP. When the customer requests a registration for less than two years (13 – 23 months), see R-180 for procedures. (Commercial vehicle, only)
  • Title: Enter the correct fee when an Alaska title is being issued. Exception: There is no charge for a duplicate title when the original was lost in the mail or as a result of divisional error.
  • Lien: (T-180) Enter a lien fee for all transactions generating a title with a lienholder. Exception: The lien fee is NOT charged for a duplicate title transaction or correction due to divisional error.
  • Tax: (R-190) Enter the amount of the motor vehicle registration tax, if applicable.
  • Inspection: (R-430) Enter the administrative fee for vehicles requiring an emission inspection certificate. The fee is charged when accepting Seasonal and Outside Use Waivers; however, it is not charged when a diesel certificate is received or when issuing a New Vehicle sticker .
  • Miscellaneous: Enter the following types of fees in this box:

    • Duplicate registration
    • License plate replacement or transfer
    • Snow vehicle/ATV decal and/or tab replacement
    • Specialty plate fees

The remaining boxes in the "For Division Use Only" section of the application are used for additional entries deemed appropriate by the motor vehicle representative to add clarification or more detail in support of the application.

PAYMENT TYPE - Mark the box for the type of payment accepted. (CA for cash, CC for credit card, CK for check) If a customer wishes to pay using a credit card and cash or check, you are to process the payment as requested.  When you process a split payment involving a credit card, please call Fiscal to obtain instructions on how to adjust your batch so the credit card amount in ALVIN equals the amount on the credit card machine report.

DOCUMENTS ACCEPTED - You must enter the abbreviations of ALL the documents accepted in support of the transaction.

Example: If a manufacturer's certificate of origin was accepted as a supporting document, the abbreviation “MCO” must be entered in this section.

BATCH NUMBER - The batch number as assigned by the computer MUST be entered.

BATCH DATE - The date the transaction was processed MUST be entered.

PROCESSED BY - The employee processing the transaction is required to legibly enter their logon ID code. Example: AMVCAJA.
