Duplicate W-2 and 1095-C Requests for Current and Prior Years
Requests for W-2 and 1095-C reprints for Executive Branch employees should be directed to the Employee Call Center, which can be reached by phone at 465-3009, by fax at 465-6624, or by email at employeecallcenter@alaska.gov. Whenever possible, an employee should use their State of Alaska email when making their request and provide their employee ID number, as this will verify their identity for Employee Call Center staff. For the Office of the Governor, the Legislative Branch and the Judicial Branch, please contact your Human Resources office for W-2 and 1095-C reprints.
Around the first week of February through April 15, every effort will be made to process and forward all requests for the previous year’s W-2 forms to the Division of Finance the same day they are received by the Employee Call Center. The Division of Finance will make every effort to respond within 24 to 48 hours. After April 15, all W-2 requests typically will be printed and processed within three work days. All reprints will be either physically mailed or emailed from the Division of Finance, or faxed from the Employee Call Center. The Employee Call Center is also able to request W-2 forms that are older than the current tax season, but the turnaround time for such a request could be significantly longer.
The mailing address for W-2 forms is the address in the payroll system effective in mid-January. To change your address in the payroll system, active employees who have access to IRIS ESS should update their address using the ESS function. If you are not an active employee, or do not have access to a computer, then you will need to submit a signed Address Authorization Form (PDF). A signature is required for our employees' protection, as sensitive financial and other personal information will be sent to the address provided.
