- Choose how you want to search by selecting ONE of the options:
- By Location
- By Doctor Name
- By Hospital or Facility
- Fill in the blanks for your selection with the required information.
- Choose what type of provider you want to locate by selecting ONE of the options:
- Search for Doctors (and select a specialty, if desired)
- Search for Hospitals or Facilities (and select a type, if desired)
- Search All (to include all provider types)
- Click on ‘search’ to get your results or ‘clear’ to start over.
