Many of the Division of Finance's forms are interactive fill-in forms. You can fill them out online and then print them, or print the forms from the website and fill them out by hand.
When positioning the cursor on a fill-in area, the cursor will change appearance. The I-beam pointer allows you to type text; the hand pointer allows you to select a check box. Press the Tab key to move from one field to the next. You can also place your cursor in the field you want. Some fields limit the maximum number of characters you can enter and some fields will change the format once done, i.e., December 20, 1999, may become 12/20/1999. Once you're done completing and printing the form, close the browser and clear the history so your private information is not viewable on a public computer (such as one in the library).

Click on the drop-down box, select the form you need, and then click Go.
